The owner or authorized representative of the business establishment has downloaded the appropriate checklist from this website and performed self-assessment.
If self-assessment is favorable, the owner or authorized representative of the business establishment shall download the Safety Seal sticker and place the same at all entrance points. He/she shall then contact the Issuing Authority to schedule an inspection.
As applicant, I commit continuous compliance of my establishment with the minimum public health standards, including the use of StaySafe.ph, or a digital contact tracing application integrated therewith
If granted the Seal and for any reason, I receive a notification or order revoking my Safety Seal, I also commit to remove the said seal in my establishment/s or website/s upon receipt of notification/order.
